Template request

Hello friends,

I started working on a boardgame, and am using MSE to prototype my cards. After considering what I'd like to eventually fit onto the cards, I wanted to request a custom template from anyone who has the time (and coding proficiency) - essentially, the Future Sight frame, but with the Leveler text box (three divided sections are what I'm looking for.
